- Description
- Provide healthcare resources in accordance with Public Law 104-262 and 38 U.S.C. 8153. Contractor agrees to provide a Registered Dietitian to provide services at the VA Roseburg Healthcare System's Community Based Outpatient Clinic, 100 River Ave, Eugene Oregon 97404. Dietitian must be able to start on January 7, 2002. If candidate is not available to start on January 7, 2002, offeror will be determined non-responsive, if after award it is determined candidate cannot start on January 7, 2002, contract shall be terminated for cause and award shall be made to next lowest offer. Price is an all-inclusive rate of per diem, travel, and price/cost. It is anticipated that each Dietitian will require three (3) days of training prior to starting work. One (1) day of training shall be held at the VA Roseburg Healthcare System in Roseburg Oregon. The additional two (2) days of training shall be on the job conducted at the Eugene Clinic by a Roseburg Dietitian. If the Offeror is submitting more than one Dietitian to cover the terms and conditions of this contract, you shall submit a schedule along with your quote indicating the time frames that each Dietitian will be scheduled to work. Contractor shall not rotate more than one provider in a three-month period. The Dietitian will see an average of 10 patients per day in the clinic and hold group session (s) with up to 20 patients when scheduled. These classes include teaching diabetic meal planning and cardiac health. The Dietitian will be assigned to Eugene Clinic during normal business hours. The work schedule will be each Friday from 8:00 a.m. to 4:30 p.m. The dietitian will teach the diabetes meal planning class once every third month on Wednesday from 9:00 a.m. to 10:15 a.m. Changes to these schedules will be mutually determined between the Eugene Clinic Manager and the Contractor. The Dietitian will have responsibility for but not limited to: a) Education and behavior change strategies in chronic disease states, including, but not limited to: diabetes, COPD, HIV, HTN, obesity, lipid and GI disorders. b) Nutrition Assessment/evaluation/intervention in patients who are malnourished, including, but not limited to education in nutritional repletion, provision of nutritional supplements per VA/VISN policy. c) Nutrition assessment/evaluation of nutritional adequacy in-patients undergoing radiation/oncology therapy. d) Any other nutritional need per orders of the Medical Staff as authorized by the Eugene VA Clinic. e) The Registered Dietitian will participate in interdisciplinary meetings, rounds, care plan or treatment plan and development as appropriate. f) Participation in local/VISN wide quality improvement activities such as Director's Performance Measures, EPRP activities, JCAHO, etc. The Dietitian must: a. Be currently registered by the CDR (Commission on Dietetic Registration). Must provide a current copy of CDR registration card with offer; b. Speak English proficiently; c. Provide evidence of demonstrated current clinical competence to perform appropriate dietitian services; d. Provide copies of any specialty credential, if any, i.e. certified diabetes educator, CDE, or certified nutrition support dietician and/or CNSD; d. Be a US citizen; e. Maintain continuing education credits in accordance with CDR standards. Must submit a copy with offer; f. Provide evidence of current professional liability insurance; g. Provide current scope of practice/Position Description; h. Names of most current employer(s) and two peer reference contacts; i. Be a team player; j. Must be computer literate, as VA Roseburg Healthcare System utilizes a computerized patient medical record. Must be able to type 40 wpm minimum; k. Must be able to adapt educational teaching strategies and materials based on patient's age, physical/emotional, barriers to learning, cultural beliefs, personal preferences, etc. The dietitian may be required to use basic teaching technologies such as overhead projector, VCR, power point presentation, etc. l. Have two (2) years of recent clinical experience as a Registered Dietitian. Contractor must provide the following documentation along with the candidate's information: 1. Evidence of current TB surveillance status. 2. Evidence of current Hepatitis B vaccination and titer. All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA. Position Sensitivity -- The position sensitivity has been designated as Low Risk. 2. Background Investigation -- The level of background investigation commensurate with the required level of access is National Agency Check with Written Inquiries. NAICS Code 621399/$5 million applies.
