Description

Provide Registered Dental Hygiene services at the Coast Guard clinics located at Seattle, WA. to beneficiaries referred for treatment by a Coast Guard Dental Officer. This will be a personal service contract. This requirement will be accomplished utilizing Far part 13, Simplified Acquisition Procedures. The contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours a day, 24 hours a week for a total of 1248 hours per year. Period of performance will be 05 February 2001, or date of award through 31 December 2001. The Request for Proposals will be issued on or about 22 November 2000 and close on or about 02 January 2001. This procurement is a Small Business Set-Aside. The NAIC code for this acquisition is 621399, the SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the proceeding four fiscal years.
