services at the Coast Guard clinic located in Astoria Or. to
beneficiaries referred for treatment by Coast Guard physicians or the
Clinic Administrator. This will be a non-personal services contract and
medical liability insurance in the amount of $1.0 million is required.
This requirement will be accomplished utilizing procedures in Far part
12, "Commercial Items." The contractor will work weekdays during hours
established by the Clinic Supervisor, 8 hours per day, 40 hours per
week, Monday through Friday, not to exceed 2000 hours per year. Period
of performance will be 01 October 1998 through 30 September 1999 with
three option years. The Request for Proposal will be issued on or
about 19 June 1998 and close on or about 20 July 1998. Performance is
expected to begin on or about 01 October 1998. This procurement is a
Small Business Set-Aside. The SIC code is 8049 and the small business
size standard is $5.0 million average annual receipts for the preceding
three fiscal years. All responsible sources may submit a bid which
shall be considered by the agency. No telephone inquiries will be
accepted.. SPECIAL NOTE: For Minority, Women-Owned and Disadvantaged
Business Enterprises: The Department of Transportation, (DOT) Office of
Small and disadvantaged Business Utilization (OSDBU) has programs to
assist minority, women-owned and disadvantaged business enterprises to
acquire short-term working capital and bonding assistance for
transportation-related contracts. This is applicable to any eligible
prime or subcontract at any tier. The DOT Bonding Assistance Program
enables firms to apply for bid, performance and payment bonds up to
$1.0 million per contract. The DOT provides an 80% guaranty on the bond
amount to a surety against losses. Loans are also available under the
DOT Short Term Lending Program, (STLP) at prime interest rates to
provide accounts receivable financing. The maximum line of credit is
