Provide Registered Nursing services at the Coast Guard clinic located in Astoria Or. to beneficiaries referred for treatment by Coast Guard physicians or the Clinic Administrator. This will be a non-personal services contract and medical liability insurance in the amount of $1.0 million is required. This requirement will be accomplished utilizing procedures in Far part 12, "Commercial Items." The contractor will work weekdays during hours established by the Clinic Supervisor, 8 hours per day, 40 hours per week, Monday through Friday, not to exceed 2000 hours per year. Period of performance will be 01 October 1998 through 30 September 1999 with three option years. The Request for Proposal will be issued on or about 19 June 1998 and close on or about 20 July 1998. Performance is expected to begin on or about 01 October 1998. This procurement is a Small Business Set-Aside. The SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the preceding three fiscal years.
