synopsis/soliciatation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solication will not be issued. Solicitation Number: RFP 539-27-97. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 90-46 and are
applicable. PART II: Description of Services: Furnish 0.5 Full Time
Equivalent (FTE) Certified Registered Nurse Anesthestist (CRNA) for the
Base Year for an estimated 1040 hours @ $__ __ ____ per hour for a

Option Year 2 Price of $__ __ __ __. CONTRACT REQUIREMENTS: 1. SERVICES
PROVIDED: a. The Contractor shall provide the services of personnel who
possess the Nurse Anesthesia degree, are certified by the American
Association of Nurse Anesthesiology, and are licensed to practice
medicine in good standing in one of the States, Territory, or
Commonwealth of the United States or District of Columbia to perform
the requirements of this contract. b. Duties include, but are not
limited to: (1) responsibility for the entire spectrum of anesthesia
care at the VAMC which includes: (a) evaluation of patients prior to
anesthesia to assess anesthestic status and risks; (b) performance of
anesthesia including general, spinal, epidural, MAC, and regional nerve
block; (c) assessment of patients post-anesthesia to determine extent
of recovery and/or complication; (d) practice such other components of
the medical sciences as may, by common practice, become part of the
practice of anesthesia within the scope of their privileges; (e)
perform related supportive services for other departments of the VAMC,
such as airway intubation, providing assistance in developing
guidelines for use of conscious sedation in other departments; (f)
provide coverage for procedures requiring anesthesia which are
typically performed in the Post-Anesthesia Care Unit to include
electroconvulsive therapy and cardioversion, for example, when not in
conflict with the ability to support operating room cases; (g)
participate in the education program conducted by the VA or Medical
Staff necessary to assist the VAMC in maintaining overall compliance
with accrediting requirements and shall perform such other reasonable
teaching functions within the VAMC as may be requested; (h) participate
in activities of the VAMC that are designed to meet the requirements of
the Joint Commission on Accreditation of Healthcare Organizations
pertaining to improving organizational performance. (i) The anesthesia
service to be provided by VAMC will consist of up to 7 CRNA and an
M.D. Director. The CRNA supervisor, working under the direction of the
Director, will assume responsibility for scheduling hours of duty,
call schedules, operative case assignments, etc, for the Contractor
CRNA staff. Contractor staff shall be expected to have personnel
available to provide anesthesia for up to one operating room daily
which will run from 7:15 am to 3:25 pm. On other days, there may be
additional contractor personnel needed to fully staff another general
anesthesia operating room as the result of VA employee staff leave,
vacancies, etc. THe Anesthesia Section will communicate these needs to
the contractor within 2 weeks of the scheduled absence. 2.
CREDENTIALING AND PRIVILEGING: The contractor personnel shall meet all
credentialling requirements of the Cincinnati Department of Veterans
Affairs Medical Center. Contractor personnel shall act in compliance
with the By-Laws, Rules and Regulations of the VAMC Medical Staff to
include regular review of Anesthesia Staff Meeting Minutes and
Recommendations and other meetings as required by the Anesthesia
Director. 3. INSURANCE: The contractor shall attain and hold an
in-force professional malpractice insurance in an amount not less than
one million dollars ($1,000,000) during the contractural period. 4.
APPROVAL OF CONTRACTOR PERSONNEL: The Director will recommend training
when appropriate as a result of quality improvement monitor findings,
or request substitution with a more qualified candidate. 5.
RECORDKEEPING: Contractor's employee must be present at the VA facility
and must be actually performing the required services for the period
specified in the contract, or the contract cost will be decreased
accordingly during each billing cycle. 6. SEMI-ANNUAL CONSTRUCTIVE
EVALUATIONS: Contractor shall provide the COTR (to be identified) at a
minimum, semi-annual constructive evaluations based on Contractor
personnel experience during the performance of this contract.
Evaluations are not limited in content and will be used to improve the
VAMC mission. At a minimum, evaluations shall address equipment,
cooperation of VA CRNA staff, other VA support services, orientations,
effectiveness of Anesthesia Section of Surgery Service and make
recommendations for improving the delivery of patient care. 7. WORK
HOURS: a. Work hours shall be Monday through Friday, an estimated 8
hours per work day, between the hours of 6:30 am and 5:30 pm. In the
event a case may exceed the normal 8 hour work day, contractor
personnel will receive compensation as indicated in the schedule. b.
There are no on-call hours associated with this contract. c. Contractor
personnel will not be required, except in case of emergency, to furnish
services on a national holiday or beyond the normal work day as
described above. SPECIAL CONTRACT REQUIREMENTS: 1. SERVICES: a. Other
necessary personnel for the operation of the services contracted for at
the VA will be provided by the VA at levels mutually agreed upon which
are compatible with the safety of the patient and personnel and with
quality medical care programming. b. Services to be performed by
contractor personnel will be performed in accordance with VA policies
and procedures and the regulations of the medical staff by-laws of the
VA facility. c. Services to be performed by contractor personnel shall
be under the direction of the Chief Surgeon of the VAMC, Cincinnati,
OH. 2. TERM OF CONTRACT: This contract is for one year from date of
award and two pre-priced one-year option periods. The contract is
subject to availability of VA funds. No service shall be performed
after September 30 until the Contracting Officer authorizes such
services in writing. 3. QUALIFICATIONS: Personnel assigned by
contractor to perform the services covered by this contract shall be
licensed in a State, Territory, or Commonwealth of the United States or
District of Columbia. The qualifications of such personnel shall be
subject to review by the VA Chief of Staff and approval by the VAMC
Director. 4. NATIONAL HOLIDAYS: The 10 holidays observed by the Federal
Government are: New Year's Day, Washington's Birthday, Martin Luther
King's Birthday, Memorial Day, Independence Day, Labor Day, Columbus
Day, Veterans Day, Thanksgiving and Christmas. Or any other day
specifically declared by the President of the United States to be a
national holiday. 5. PERSONNEL POLICY: The contractor shall be
responsible for protecting the personnel furnishing services under this
contract. To carry out this responsibility, the contractor shall
provide the following for these personnel: worker's compensation,
professional liability insurance, health examinations, income tax
withholding, and social security payments. The parties agree that such
personnel shall be not consideredVA employees for any purpose and
shall be considered employees of the contractor. The solicitation

Nondiscrimination in Services Provided Beneficiaries. BASIS OF
CONTRACTOR SELECTION: The Government will award a contract resulting
from this solicitation to the responsible offferor whose offer
conforming to the solicitation will be most advantegous to the
Government, cost or price and other factors, considered. The Government
may award a contract on the basis of initial offers received, without
discussions. Therefore, each initial offer should contain the offeror's
best terms from a cost or price and technical standpoint. EVALUATION
FACTORS FOR AWARD: 1. Proposals will be evaluated based on the
following factors in order of relative importance (i.e. (a) is more
important than (b); (b) is more important than (c); etc....FACTOR (a):
Quality of Service; including Past Performance; FACTOR (b):
Organization and Personnel of Offeror; FACTOR (c): Technical Approach;
FACTOR (d): Price. SUBMISSION OF TECHNICAL PROPOSAL: Offerors are
required to submit 3 copies of their Technical Proposal. The Technical
proposal shall not contain any information relative to pricing. Price
proposals shall be submitted in separate binders from the technical
proposals. Technical proposals shall address the contract requirements
and the ability of the contractor. The Technical Proposal shall
address the Evaluation Factors for Award and shall be concise and
relevant to those factors. INCLUDE A COMPLETED COPY OF 52.212-3 OFFEROR
REPRESENTATIONS AND CERTIFICATIONS -- COMMERCIAL ITEMS; 52.215-6 TYPE
