36C25225B0001 0003 SF30 Continuation Sheet ACCOMPANYING 36C25225B0001 0003 SPS Renovation Construction Phases 2, 3 & 4 project#537-17-115 Jesse Brown VA Medical Center, Chicago, IL I. The following changes are to be made to the solicitation: 1. THE BID OPENING IS HEREBY CHANGED FROM IN-PERSON TO VIRTUAL. 2. THE BID OPENING DATE IS HEREBY EXTENDED FROM MARCH 17, 2025 AT 10:00AM TO MARCH 24, 2025 (see additional information below in red) 3. ADD RYAN McMILLIN TO THE point of contact OF REQUEST FOR INFORMATION (RFI) AND BID RECIPIANTS AT RYAN.McMILLIN@va.gov a. VIRTUAL BID OPENING INFORMATION: 1) Reminder, there will be no in-person public bid opening process. This will be conducted as a public virtual activity. 2) The virtual bid opening will be available to the public via Microsoft Teams at the following link and/or phone number: Join the meeting now (Meeting will be available to be accessed 15 min prior to bid opening) Meeting ID: 243 355 189 787 Passcode: B2gJ7Ae7 Dial-in by phone: 872-701-0185; Phone conference ID: 699 467 033# 3) IAW FAR 52.214-5 Submission of Bids and FAR 14.202-8 Electronic Bids; Bids shall be provided via email to the Contracting Officer at Stacy.Hoover2@va.gov AND Ryan.McMillin@va.gov as follows: BIDS DUE: Vendor bids are due via email as shown above at 10:00 AM local time on 3/24/2025. The virtual public bid opening will be held at 11:00 AM local time on 3/24/2025 at the link and/or the phone # provided above. A hard copy submission is not required. Any bids received after the date and time specified for receipt of bids will be processed in accordance with FAR 52.214-7 Late Submissions, Modifications, and Withdrawals of Bids (Nov 1999). 4) Ensure your complete bid including bid bond are provided in your submission. The successful low bidder is required to furnish their original bid bond within five (5) days after notification by the Contracting Officer. 5) Ensure your bid includes the completed VAAR 852.219-75 VA NOTICE OF LIMITATIONS ON SUBCONTRACTING CERTIFICATE OF COMPLIANCE FOR SERVICES AND CONSTRUCTION (JAN 2023) (DEVIATION) found on pages 67 and 68 in the original SF1442 solicitation document for reference. 6) It is highly recommended that Bidders submit their bid at least one hour prior to the bid due date/time to avoid any transmission issues. II. The following changes are made on project specifications: n/a III. The following changes are made on project drawings: n/a IV. The following Questions and Answers / Clarifications are the result of bidders request(s) for information: Please confirm if the superintendent can be SSHO and QCM. 1a. Yes. As long as the superintendent has the necessary qualifications and experience to manage safety and quality control responsibilities on-site. Please confirm if the VA will be hiring the commissioning agent directly. 2a. Yes. VA has contract in place with commissioning services. What are the construction hours? Are we able to start as early as 6:00 am? Please confirm any areas that require off-hour work for each phase. 3a. A 6:00am start time is acceptable. Work in all phases can generally be performed during the day M-F. However, per the SOW work that generates significant noise, odor, vibration (e.g. core drilling) and any utility shutdowns that are not localized to just the construction area must be performed off-hours to minimize impact to the facility and without cost-increase to the VA. Are contractors able to provide trailers and/or storage boxes?� 4a. Not at this time Will the VA provide laydown/storage areas? If so, where will they be located?� 5a. There is limited space available on 3rd floor of bed tower. Contractors can also store within the construction zone. Will the VA provide an area for a dumpster? If so, where will that be located?� 6a. Space for a dumpster is not available for the full duration of the project. However, during demolition the COR / station will make every effort to free up space in one of the dock areas - however space is very limited and the dumpster location must be coordinated with the COR to prevent interference with deliveries and pick-ups. Are CAD files available for all of the issued drawings?� Or will they be available after award is made? 7a. CAD files will be made available after award. Reference Specification 010000, Section 1.28 & 29: Please confirm that the photographer must be 3rd party contractor outside of GC s company, or can be performed by GC personnel. It lists Monthly, Weekly, and Regular photographic documentation are we required to have professional photographer provide the monthly? The weekly? And the regular progression photos? There are costs associated with this work, so please clarify specifically. 8a. This spec section is not applicate as this is not a Major construction project. GC personnel are expected to take photographs for daily logs, inspections, RFI etc. But, professional photography is not required. Reference Specification 010000, Section 1.31 Is there a cost for the use of VA Tririga CPMS? Will there be training required or provided for use of this system? Are we able to propose a separate project management software option, if GC provides? 9a. This is not a major construction project, therefore the Tririga CPMS requirement is not applicable. Project Management Software Submittal Exchange to be used - VA will provide the software access (i.e. No cost to the GC / no licensing fee for this software). Please confirm requirements for required onsite personnel when work is being performed. Refer to specifications 013526 and 014000. Is the GC required to have 1 or 2 onsite personnel full time while work is being performed? We interpret that we are required to have (1) SSHO/Safety onsite when work is being performed. Is the (1) QCM as a separate individual, required onsite while work is being performed? Can the QCM also serve as the site superintendent? This affects project costs. Please confirm. 10a. Please see response to question #1. Duties of the SSHO and QCM can be performed by the same person (e.g. the superintendent) provide that person has the necessary qualifications and experience to manage safety and quality control responsibilities on-site. Will the VA be providing a 3rd party commissioning agent for this project? If yes, will they have separate documentation required to be completed prior to project being complete? 11a. See response #2 - Yes, VA will provide commissioning agent. Specification 078400 Firestopping, please confirm that any inspectors of fireproofing are required to be independent 3rd party contractor and ASTM E699 certified. 12a. Yes, contractor to engage a qualified inspector to perform firestop inspections and final reports. V. general information: 1.
