- Amendment No. 1 to Combined Synopsis/Solicitation RFQ No. 36C24825Q0285 The amendment is hereby issued to provide responses to questions and request for information submitted by possible vendors. Quote/Offer due date remains unchanged. All other areas of solicitation remain unchanged. RESPONSE TO VENDORS QUESTIONS AND REQUEST FOR INFORMATION Q1. Is this a new RFQ or is there any existing contract in place that provides this service, and this is just a rebid of a previous contract. If the answer to question is yes please provide contract number in place at this time. Yes, there is an existent contract in place, as this is a follow-on contract being procured as a competitive set-aside for SDVOSB verified vendors. Current contract number is 36C24820D0037. This information is publicly available on Federal Procurement Data System (FPDS) at www.fpds.gov and/or US Spending at www.USASpending.gov Current contractor information may be requested and disclosed under Freedom of Information Act (FOIA) in accordance with 38 Code of Federal Regulations (CFR) 1.553(b). The VA accepts FOIA requests submitted by email. Email must be sent to SAJFOIA@va.gov . For additional information about FOIA regulations and procedures, please visit www.foia.va.gov Q.2 Pricing/Cost Schedule (4 - 6 of 80 in solicitation) ask for drops in USVI. We are familiar with area but there is only (1) outpatient clinic each in St Thomas and St Croix shown as locations served on page 19 of solicitation is this correct? Yes, the VA Clinics in U.S. Virgin Islands are the ones in St. Thomas and St. Croix as stated on page 19. Quoter/Offeror shall provide a single rate/price for services on any of those two locations. Q.3 Page 11 of solicitation 2.1.1 states a 24-hour response time. Page 16 Item. E number 1 and 2 state regular work to be scheduled and accomplished a week in advance and emergencies responded to within 1 business day. These statements are ambiguous Please clarify The emergency response for on-site support will be as stated on Page 16, Item E, regular work to be scheduled and emergencies responded within 1 business day. Page 11, 2.1.1 is to be edited to reflect the same information as Page 16, Item E. See Attachements-36C24825Q0285- Cable Drops Installation Services at VACHS- Rev 1, which incorporates the change. Q.4 Will the VACHS be able to provide small storage area at 10 Casia Street and other larger Clinics in order to reduce overhead for the contractor so they might pass savings on to VA. No, the government will not be providing storage area. The contractor is responsible to perform services by bringing their own materials and equipment as well as taking them at the end of each day. Q.5 Pages 55 57 State Evaluation Procedures but there is no SF 1449 or SF 1442 in solicitation package to be submitted at this time. Please clarify. If the answer to question is that no Offer and Award Documents are included presently then the documents provided are FACTORS 1,2 and 3 only. Please clarify. This is a combined synopsis solicitation per FAR 12.603 Streamlined Solicitation for Commercial Products or Commercial Services, in which the announcement and the solicitation are posted at the same time and no SF1449 is required. Submit your Quote/Offer and required documentation as stated on the addendum to 52.212-1. Q.6 It is our understanding that Reps and Certs are to be turned in with Evaluation documents in the initial submission. Please clarify. Yes, all documents are to be provided in the initial submission. Q.7 Does a Blanket Purchase Agreement allow to provide prices for services/products repetitively like in this case for five (5) years? Yes, vendors quote/offer shall price all items on each period, base and options as stated in solicitation. Q.8 In the case of Virgin Islands are travel expenses to be included in price? Price to be quoted for each line item should be an all-inclusive price. This is a performance-based requirement, the Government expects Offerors to thoroughly review the PWS and submit their best quotation/offer based on their experience, and commercial practices.
