- PERFORMANCE WORK STATEMENT (PWS) GENERAL The contractor shall provide services for the U. S. Naval Medical Research Unit No. 6 (NAMRU-6) in Puerto Maldonado (Amazon region), Peru during the period from May 18, 2020 to May 17, 2021. Services will be paid monthly. Services will be provided 40 hours per week from Monday to Friday. Working hours will be coordinated with the Facilities Department in the range of 0700 to 1800. Services consist of recruiting, training and placing personnel for the services detailed below. Contractor must receive Government approval before personnel is selected for placement. The contractor shall furnish all managerial, administrative, direct labor personnel and uniform that are necessaries to accomplish all work required. Contractor employees shall be on site only for performance of contractual duties and not for other business purposes. Performance requirements for required work are described below. The contractor is responsible for strict adherence to all instructions and quality requirements stated in this contract and shall provide the appropriate management effort to ensure that all services are performed. The contractor shall ensure that all personnel assigned to this contract possess the skills and experience necessary for meeting deliverables. The services shall include the laborer to work in a laptop with internet service and work cellphone with cell service and to have a small printer/copier/scanner necessary to accomplish all work required. Laptop will be used to email quotes and/or receive standard operation procedures to perform work and/or trainings. Work cellphone shall have camera in order to take pictures and report deficiencies. Laptop to be selected and provided by contractor to run MS office minimum and adobe acrobat reader. Preferable to be Hewlett Packard or Dell. Since laborer shall send reports via email, contractor shall provide portable internet for this laptop. Contractor to consider also maintenance and software updates for this equipment. Contractor to provide office paper to print documents/reports (estimate of a box of 500 sheets per month) and any other office materials (pen, pencil, etc.) necessary to accomplish all work required. Government will not provide cellphone, laptop, printer, cell service, internet service, or maintenance for any communication equipment at location. The services requested do not include any other materials and tools or equipment in order to perform the cleaning and maintenance tasks. All other materials, equipment and tools required for the correct execution of the work described below will be provided by NAMRU-6. CONTRACTOR RESPONSIBILITIES The contractor is responsible for strict adherence to all instructions and quality requirements stated in this contract and shall provide the appropriate management effort to ensure that all services are performed. The contractor shall ensure that all personnel assigned to this contract possess the skills and experience necessary for accomplishing their individual tasks. The Contractor must provide the name of the person of contact (POC) and his/her alternate in charge of daily communication with NAMRU-6. If these employees are going to be changed, NAMRU-6 must be notified in writing three working days in advance. Contract requires for continuity of services during the duration of the contract. Employee(s) working for the Contractor in NAMRU-6 facilities have to be cleared by Regional Security Office (RSO) at the U.S. Embassy, Lima, Peru. Personnel without a clearance check will not be allowed to provide services required per this contract. The contractor shall ensure that their personnel assigned to provide the services required per this contract are in compliance with the local labor regulations. The services required per this contract are stated below: LABORER -�PUERTO MALDONADO (Amazon region) Laborer services (one laborer) are required to provide support to our laboratory in NAMRU6-Puerto Maldonado (remote site) by: 1. Performing minor repairs to electrical systems (generator, electrical panels, etc.). Laborer shall evaluate the condition of the facilities and reports of deficiencies�������������.30% 2. Laborer shall act as an escort and supervise contractors during preventive and corrective maintenance of the systems of the Facility (air conditioning, plumbing, electrical, etc.)����. 30% 3. Laborer shall perform safety tasks assigned by the Safety Officer and the Environmental Officer; such as: monthly inspections of the fire extinguishers, monthly inspections of AED, monthly inspections of eye wash station and perform related duties as required.����������....10% 4. Laborer shall coordinate with garbage truck to pick up hazardous waste���������10% 5. Laborer shall participate in moving when required of heavy furniture and/or laboratory equipment. Laborer shall coordinate with courier for all delivers except confidence materials. Laborer shall register all material received and delivered by the courier service. Laborer shall move supplies to proper storage areas, using hand truck and other material handling equipment���������������10% 6. Laborer is responsible for reporting broken windows, stopped up drains, and leaking plumbing and for securing premises after workday is completed. Laborer shall clean floor and furniture of laboratories and any other assigned area����������������������� 10% The Contract will include round trip transportation service due to the NAMRU-6 is located about 3 km of the City downtown, as attached in the below list:. Completion of high school education is required. Technical training in Electricity is required (400 hours minimum). Laborer shall be fluent (Level III) in Spanish.
