THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS   The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work require under the proposed contract. For purposes of being considered a small business, the following applies: NAICS Code 621399, Size Standard $7.5 million. DESCRIPTION OF THE REQUIREMENT: The Network Contracting Office 4 is conducting market research for the Department of Veterans Affairs Pittsburgh Healthcare System (hereinafter referred to as VAPHS) to identify potential sources for a contract to provide onsite Cardiac and Liver Perfusion Services to eligible beneficiaries of the VAPHS on an as needed basis 24 hours a day, seven days a week, including holidays. Services include but are not limited to: Scope of Care: Contract personnel (s) (as appropriate and within scope of practice/privileging) shall be responsible for providing Cardiac/Liver Perfusion care as well as other modalities such as ventricular assist device support and extracorporeal membrane oxygenation (ECMO) support, including, but not limited to : All Perfusionists shall assist the Cardiac Surgery service with by-pass surgical cases and valve replacement of various types. The contractor shall perform onsite Cardiac/Liver Perfusion Services in the treatment of care for eligible veteran beneficiaries as referred and authorized by the VA Pittsburgh Healthcare System, University Drive VAMC. The Perfusionists are responsible for preparation, and operation of the heart and lung machines/pumps in conjunction with maintaining a sterile environment for procedures maintaining blood volume in the patients preventing shock or air embolus. Assemble supplies and equipment in preparation to accept patient on extra corporeal circulation in cases of premature cardiac arrest or failure. Other services include preparation, maintenance, cleaning and operation of VA owned equipment to include the heart lung machine, auto transfusion pump, intra-aortic balloon pump and centrifugal ventricular assist device. The Cardiac surgeon attending will oversee the activity of the Perfusionists during cardiac surgery. The contractor will assign Perfusionists who have knowledge of Cobe, Haemonetics, BioMedicus, Sarns and Datascope equipment, as well as the ability to adapt additional equipment as necessary. Perfusionists will be familiar with the operation of cardiopulmonary bypass equipment so that minor repairs may be made on an emergent basis in the event of equipment failure during a procedure. The VA Pittsburgh Healthcare System Biomedical Department is responsible for major and non-procedure occurrence repair and maintenance (other than cleaning) for all VAMC-owned cardiopulmonary and perfusion-related equipment. The contractor will designate a Perfusionist to act in the capacity as Charge Perfusionist for the administrative services provided by the contractor. Perfusionists assigned by the contractor will have the ability to: Set up and operate cardiopulmonary bypass equipment, conduct cardiopulmonary bypass, monitor and document all patient perfusion parameters, and make necessary adjustments for routine and emergent procedures requiring cardiopulmonary support. Operate all associated perfusion equipment including cardioplegia, cell saver, intra-aortic balloon device (IABD), femoral-femoral cardiopulmonary support (emergency bypass), extracorporeal membrane oxygenation (ECMO) as necessary. Administer medications and blood components via bypass as necessary after consultation with the attending cardiac surgeon or anesthesiologists. Assist in trouble-shooting IABD problems. Operate veno-venous bypass machine for liver transplant procedures. Clean all perfusion equipment at the end of the procedure. Arteriocyte-Magellan Bone Marrow Aspirate Concentrate (BMAC). Coverage Period 24 hours per day 7 days per week with response time commitment 1 hour. Set up and confirmation of device function Assist anesthesia provider with whole blood phlebotomy Assist surgical technologist and surgeon with bone marrow aspirate draw Prepare BMAC Dispense and support surgical technologist and surgeon with applicator preparation and administration of BMAC Provide patient record of procedure Pending VA administration decision, support quality improvement program to include lab sampling and reporting of performance indicators Ensure adequate inventory levels are maintained by coordinating the request for supply replacements with VAMC Operating Room Staff through VAPHS Logistics personnel as may be needed. Promote patient safety before, during, and after the operation. Maintain aseptic technique. Assist in the evaluation of new products and procedures. It is the Perfusionists responsibility to view the patient record within CPRS prior to the surgery to review the surgeon s plan of care. Contractor will assign Perfusionists to provide continuing education for VA Pittsburgh Healthcare System Operating Room Staff as necessary, i.e., as technology changes, techniques change, etc. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements to Bruce Jackson no later than 2:00 PM EST on March 21, 2018.
