Description

This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 532291, Size Standard 7 Million. The Department of Veterans Affairs Upstate New York VA Healthcare has a requirement for the rental, delivery and respiratory therapy services associated with Home Oxygen Services for Veteran patients of VISN-2. Home Oxygen Services will be provided to beneficiaries of six medical centers and numerous outpatient clinics in Upstate New York. The facility locations that comprise VISN-2 can be viewed at http://www.visn2.med.va.gov/. Area of service also incorporates additional states bordering VISN-2 (ie: PA, NH, MA) that the some Veteran Beneficiaries reside in but utilize the services of VISN 2 Medical Centers. The Contractor shall furnish all rentals, delivery, supplies, oxygen, service of oxygen equipment, ventilators, delivery and service of Government provided sleep apnea equipment. Contractor shall also provide New York State (PA, NH, MA, etc.) Licensed Registered Respiratory Therapist Services to train educate and monitor patients. The performance period will be on or about August 1, 2012 - July 31, 2013 with four option years that may be exercised at the sole discretion of the government. This procurement is being conducted under FAR Part 12 and FAR Part 15. Responses to this sources sought must include the following information. 1. General information of your company name, address, contact person name, phone number, fax number, email address(s) and DUNS number and company website if available. 2. Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged Business, Service Disabled Veteran Owned small business or Veteran Owned Small Business. 3. Does your company have experience with a similar program? 4. What does your company primarily provide? 5. Will your company be a joint venture with another company? 6. How long has your company existed? Responses to this market survey should be emailed to Allen.Johnson2@VA.GOV. No telephone responses will be accepted. All Veteran Owned Business must be registered in VetBiz at http://www.vip.vetbiz.gov/general_user/register/default.asp to be considered a Veteran Owned Business. Responses must be received no later than 3:00 pm on March 21, 2012. No solicitation document is available at this time; this notice is to acquire information only. Vendors interested in providing an offer will have to respond to a separate solicitation announcement.
