Description
The VA VISN 19 Rocky Mountain Health Care Network, Glendale, Colorado intends to negotiate a commercial (FAR Part 12) sole source procurement with Total Longterm Care, Incorporated, 200 East 9th Avenue, Denver, CO 80203-2903 under the authority of FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements.     The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12.     The Request for Proposal is VA-259-07-RP-0019.     The North American Industry Classification System Code (NAICS) is 624120 and the small business size standard is considered $6.5M.     A justification for other than full and open competition has been prepared in support of this acquisition.     The Contractor will provide a comprehensive long term care benefit package of both traditional Medicare and Medicaid services, including any home and community based services as defined by the Colorado State Medicaid Plan for eligible veterans residing in the Contractor's service area. Services will be provided either at the client's home, or the Daycare. The treatment team shall consist of Primary Care Physicians, Case Managers, Home Care Coordinators, Transportation, Rehab Services, Registered Nurse, Registered Dietician, Activity Coordinator, and Social Worker. These services will be provided with no deductibles, co-payments or limits to services to the veteran. All care is to be authorized in advance, except for emergencies, by the multidisciplinary team. Services required are as a result of the pilot programs undertaken by Public Law 106-117, The Veterans Millennium Health Care and Benefits Act, and Section 102 and Public Law 104-262 and 38 USC 8153. No solicitation document exists and no telephone inquiries will be accepted. All interested persons may identify their interest and capability to respond to the requirements. THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. The Government will consider all proposals received by 1:00 PM Mountain Standard time November 3, 2006. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
