Description
The Acquisition Department, Fleet and Industrial Supply Center (FISC), Norfolk, VA, 23511-3392 intends to issue a Firm Fixed-Price Requirements contract for Home Oxygen Services for approximately eighty (80) patients as requested by the Naval Medical Center (NMC), Portsmouth, VA. The services required shall consist of a base year with four one-year renewable option periods. The effective base period is projected to be 1 December 2002 through 30 September 2003. A complete Statement of Work (SOW) will be included in the solicitation. The contractor shall have an established place of business within a fifty- (50) radius of NMC Portsmouth, VA and shall be able to provide services twenty-four (24) hours a day, seven (7) days a week including holidays for emergencies. Response times to emergency calls shall be within four (4) hours. The contractor shall maintain accreditation by the Joint Commission on Accreditation of Healthcare Organization (JCAHO) and shall maintain such accreditation throughout the duration of the contract. The contractor shall provide the Contracting Officer a copy of the JCAHO certification and following re-certification during the contract period. If accreditation is lost during the contract period, the contractor shall immediately notify the Contracting Officer. The contractor shall permit the Contracting Officer and Technical Representative(s) to conduct on site visits to the contractor's facility after the closing date of the solicitation, but prior to award of the contract. (See numbered note 23). The North American Industry Classification System (NAICS) code for this acquisition is 621610. This procurement has been set-aside one hundred (100) percent for small business concerns. (See numbered note 1). All qualified small business offerors are encouraged to submit an offer. Potential offerors are hereby notified that the solicitation and subsequent amendments to the solicitation will only be available by downloading the documents at http://www/neco.navy.mil. This office will no longer issue hard copy solicitations. By submitting a proposal, the vendor will be self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any Federal Agency. Failure to do so may represent grounds for refusing to accept the proposal.
